The hidden revenue in proper care planning reviews

Care plan reviews are often treated as administrative obligations rather than clinical encounters. That framing costs practices more than they realise.

A proper care planning review is not a tick-box exercise. It involves reassessing patient goals, evaluating progress, identifying barriers, and adjusting management where needed. When done well, it improves care continuity and supports appropriate Medicare billing.

In many practices, however, reviews occur informally. They are folded into “quick check-ins”, nurse visits, or opportunistic GP appointments without clear intent or structure. The work still happens, but its clinical value is diluted and the billing opportunity is lost.

The issue is not lack of effort. It is lack of clarity.

Practices that perform well treat reviews as deliberate clinical events. They schedule them, document them clearly, and ensure they link back to the original management plan. Nurse involvement is purposeful and supervised, not incidental.

This does not require longer appointments or more resources. It requires reframing reviews as part of active management rather than paperwork.

When reviews are documented properly, they demonstrate continuity of care, clinical reasoning, and patient engagement. Medicare billing then reflects care that is already being delivered, rather than creating additional work.

Care planning reviews are one of the most under-optimised areas of general practice. When approached thoughtfully, they benefit patients, clinicians, and practice sustainability.
